Police are appealing for witnesses following a fatal road traffic collision on the M4 in which 73-year-old woman died.

Police units were on patrol on the westbound M4 motorway, between junctions 3 and 4, on Sunday, 26 September at approximately 09:35hrs when they discovered the collision between a silver Volkswagen Golf and a blue Honda Jazz. Both vehicles had left the carriageway and come to rest in a drainage ditch. Two other cars - both Volkswagen Golfs - had also stopped at the scene.

London Ambulance Service and HEMS (air ambulance) attended the scene and a 73-year-old woman, the driver of the Honda, was taken to Hillingdon Hospital suffering injuries - she died on Monday, 26 September.

A post mortem took place at Uxbridge Mortuary and gave cause of death as head injuries. An inquest opened under the West London Coroner's office at Fulham Coroner's Court.

Next of kin have been informed and the deceased has been formally identified as Frances Cann, aged 73 years, from Hammersmith.

Three male drivers were arrested at the scene and later released on bail to return to custody in early November.

Officers from the Road Death Investigation Unit based at Hampton Traffic Garage are keen to hear from anyone who may have witnessed the collision, or the movements of the cars prior to this.
